Register, Schedule Appts and Make Payments

 Click HERE, to register as a new client, schedule an appointment or make a payment.

The steps to register and schedule are as follows:

1. Click on the above link to access my registration portal.
2. Click on “register as a new client” to enter your information. Be sure to add any insurance ID# and group#.
3. Create a username and password.
4. Click on the link to see all my available appointment.
5. Select an appointment time.

To make a payment:

1. Log in with your username and password.
2. Click on make a payment.
3. Scroll down and click on “view or pay online statement.”
4. Follow instructions to complete the payment.
